CREATE Medicare Information Materials
Medicare is a complex system that may be confusing to navigate. The CREATE team has developed the Medicare Information Series and Decision-Making Flowchart to aid understanding. These resources can be used by older adults, their family members, SHIP counselors, or anyone assisting with Medicare decisions. Keep in mind that Medicare information may change over time. The Medicare.gov website provides the latest and more detailed information.
We have created different versions of the materials. Versions for Illinois and New York have specific contact information for each state. The generic version has a space for you to fill in the details for your state.
